お子様の氏名、生年月日等、保護者記入欄は、あらかじめ全てご記入ください。（鉛筆、シャープペンシルは不可）記入のない場合は、お受けできません。
please pre-fill-in all forms prior to consultation, including your child's name, date of birth, signature of parent(s) in black ball-point pen
診断書等所定の様式があればお持ちください。

please note that we neither perform hearing tests with audiometer nor a dental check
please book with enough time in advance to allow us a week maximum to finalize all certification before you actually need to use them - thank you

母子健康手帳をご持参ください。オージオメーターによる聴力検査はできません。歯科健診は行っておりません。内容によっては、当日作成できないこともありますので、日程に余裕をもって受診してください。
